ARA290
ARA290 (Cibinetide) is a synthetic 11-amino acid peptide derived from the helix B domain of erythropoietin (EPO). Specifically, it was engineered to selectively bind the innate repair receptor (IRR)—a heteromer of EPO receptor and $\beta$-common receptor ($\text{CD131}$) subunits—without activating the homodimeric classical erythropoietin receptor ($\text{EPOR}_2$) responsible for erythropoiesis.
In contrast to intact erythropoietin, this high receptor selectivity allows ARA290 to trigger cytoprotection without increasing hematocrit or red blood cell mass. Mechanistically, research into ARA290 has focused primarily on its ability to mitigate neuropathic pain and accelerate peripheral nerve repair through IRR engagement on sensory neurons and surrounding immune cells.
Consequently, preclinical and clinical studies have evaluated its efficacy in small fiber neuropathy models, demonstrating that treatment can reduce neuropathic pain scores, increase intraepidermal nerve fiber density in cutaneous biopsies, and improve metabolic parameters in diabetic neuropathy. In addition, researchers have characterized its broader anti-inflammatory actions, which operate by modulating macrophage polarization away from pro-inflammatory phenotypes and dampening systemic cytokine signaling. Furthermore, its ability to selectively stimulate the tissue-protective arm of the EPO axis avoids the thrombotic risks associated with classical erythropoietin administration.
